1. ASSUMPTION OF RISK I, the undersigned, understand that participation in Annie Bonsignore Studios Summer Camps involves activities including, but not limited to: singing, dancing, playing musical instruments (ukulele, piano, percussion), arts and crafts (using various materials), and supervised outdoor play. I am aware that these activities involve inherent risks, including the risk of minor physical injury, slips, trips, falls during movement or outdoor play, and exposure to common allergens. I voluntarily assume all risks associated with my child’s participation in these activities. 2. RELEASE OF LIABILITY AND WAIVER In consideration of my child being permitted to participate in the camp, I hereby release, waive, and forever discharge Annie Bonsignore Studios, its owner (Annie Bonsignore), employees, volunteers, and agents (collectively, the "Releasees") from any and all liability, claims, demands, or causes of action for any injury, loss, or damage to person or property arising out of my child’s participation in the camp, whether caused by the negligence of the Releasees or otherwise, to the fullest extent permitted by the laws of Ontario. 3. INDEMNIFICATION I agree to indemnify and hold harmless the Releasees from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, brought as a result of my child’s involvement in the camp and to reimburse them for any such expenses incurred. 4. MEDICAL EMERGENCY AUTHORIZATION In the event of an illness or injury, I hereby authorize Annie Bonsignore Studios staff to seek medical treatment for my child and/or to transport my child to a medical facility if I cannot be reached immediately. I agree to be responsible for any costs associated with such medical treatment or transportation. 5. PHOTO / VIDEO RELEASE I consent to Annie Bonsignore Studios taking photos and videos of my child during camp activities and performances for use in studio promotional materials, social media, or the studio website. I understand that no first or last names will be used. 6. GOVERNING LAW This agreement shall be governed by and construed in accordance with the laws of the Province of Ontario. If any portion of this agreement is held invalid, the remainder shall continue in full legal force and effect. I HAVE READ THIS RELEASE OF LIABILITY AND FULLY UNDERSTAND ITS TERMS. I AM AWARE THAT BY ENROLLING IN THIS CAMP, I AM WAIVING CERTAIN LEGAL RIGHTS.
